We are happy to provide you the service of completing and submitting your insurance forms with all the information needed to process your claim. This will help you receive the maximum benefit payment available under your policy. We will also accept assignment on your behalf with certain exceptions. Cash, Check or Credit CardWe are happy to offer a courtesy discount for all treatment that is paid in full with cash, check, or credit card prior to the onset of treatment. We accept payment by MasterCard, VISA or Discover. Automatic Preauthorized Bank DraftWith your authorization, your monthly payment will be wired directly from your account electronically on the 25th of each month. Automatic Preauthorized Credit Card PaymentWith your authorization, your credit card is automatically charged on the 10th of each month for the amount of your monthly payment based on your financial agreement.
